Working with the patient’s physician, getting well at home is possible with an interdisciplinary home health care team of visiting nurses, therapists, caregivers and social workers. VNA Hospice & Palliative Care is a non-profit provider of comprehensive care and support to patients and families facing the challenges of life-limiting illness and loss. Care is coordinated among a team of clinicians including medical doctors, nurses, hospice aides, social workers, chaplains and trained volunteers. Support can take many forms; medical, physical, emotional, practical and spiritual, ensuring help is provided to all involved. Many patients receive hospice care in their homes, whether that's a private home or apartment, or in a nursing home or skilled nursing facility.

Eligibility for Pemi-Baker services is determined by a patient’s physician. This care is in conjunction with active treatment for a disease. The palliative care team focuses on the pain and discomfort a patient may feel while undergoing treatment while your primary doctor and specialist focus on treating your illness. While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health since October 2021. When I retired, I began searching for volunteer work that would allow me to give back to my community.
Bereavement support and counseling are also offered to families and loved ones. For more than a century, VNA Health has been the community’s leading nonprofit provider of home health care. Your VNA Health Home Health Care Team works with your physician to build a customized care plan to help you recover and regain your independence. Our Home Health Aides provide personal care (bathing, shampooing, simple dressings, etc.) and may perform some light housekeeping for patients who are unable to do so. Home health aides provide non-nursing services like bathing, personal hygiene, therapeutic care and light housekeeping. Provides functional supervision of all Home Health/Home Care Staff on weekends .
In 2016, VNA HEALTH AT HOME charged a total of $22,412,946 to Medicare for a total of 7,404 (non-LUPA) home health episodes provided to 4,720 Medicare beneficiaries. Licensed Physical Therapists will, upon request of the physician, assess a patient’s need for therapy. They work closely with the physician and other home health staff, as well as the patient, to develop a plan of treatment suited to the patient’s needs. Our team serves those with or without family support and those who may have limited financial resources. With the assistance of United Way of the Midlands, we are able to care for individuals who are uninsured or otherwise unable to afford or access services.
